Book Part Acil Servis Kalabalıklığı ve Yönetimi(Nobel Akademik Yayıncılık, 2023) Bütün, AhmetAcil servislere başvuru sayısı gün geçtikçe artmakta ve acil sevişlerde hasta yoğunluğuna sebep olmaktadır. Acil servis kalabalıklığı hem ülkemizde hem de dünyada kronik bir sorun olarak karşımıza çıkmaktadır. Acil servis kalabalıklığı, acil servislerin işleyişini ciddi bir şekilde etkileyen ve hâlâ çözülmemiş bir problemdir. Acil servis ziyaret sayısı giderek arttığı için bu sorun hem Türkiye’de hem de dünyada önemli bir halk sorunu hâline gelmiştir. Acil servislerin aşırı yoğunluğu, acil servis personelinin yaşadığı en büyük ve en önemli sorunlardan biridir. Acil servisler üzerindeki baskı, acil servis personeli, acil servis yöneticileri ve politika yapıcılar için dünya çapında önemli bir sorun olarak durmaktadır. Acil servislere yapılan ziyaretlerin önemli bir kısmını durumu acil olmayan hastalar oluşturmaktadır. Bu hastaların acil servislere başvuruları acil servislerde aşırı hasta yoğunluğuna yol açmaktadır. Böylece acil servislerde bekleme süreleri uzayabilmekte, ciddi hastalığı olan hastaların tedavileri gecikmekte, hasta memnuniyetsizliği artmakta, acil serviste genel bir karmaşa ve yetersizlik durumu ortaya çıkmaktadır. Acil servislerin aşırı yoğun olması, acil bakıma en çok ihtiyaç duyan hastaların tedavilerinde gecikmelere neden olmakta, verilen sağlık hizmetlerinin kalitesini düşürmekte ve sağlık hizmeti maliyetini artırmaktadır. Bu kitap bölümünün amacı acil servis kalabalıklığının sebeplerini tespit etmek ve bu kalabalıklığı azaltmaya yönelik çeşitli çözüm önerileri sunmaktır.Article Analysis of bioactive compounds using LC-ESI-MS/MS, cytotoxic, antimicrobial effects, and enzyme activities from Cyclotrichium origanifolium(Wiley Online Library, 2022) Aktepe, Necmettin; Keskin, Cumali; Atalar, Mehmet Nuri; Baran, Mehmet Fırat; Keskin, Cumali; Taşkin, Abdullah; Yavuz, Ömer; Demirtaş, İbrahim; Oğuz, Ercan; Jahan, IsratCyclotrichium origanifolium is a medicinal plant belonging to the Lamiaceae family. In this study, phenolic content analysis, antimicrobial effects, and cytotoxic effects of extracts of C. origanifolium were investigated. In the extracts, phenolic compound analysis by the liquid chromatography–electrospray ionization– tandem mass spectrometry method, antimicrobial effect by the minimum inhibition concentration method, and cytotoxic effect on human dermal fibroblasts (HDF), glioblastoma cell (U87), ovarian adenocarcinoma cell (Skov-3), and human colorectal adenocarcinoma cell (CaCo-2) cancer cell lines were investigated. Cytotoxicity analyses were performed by the MTT method. In addition, the GST and AChE enzyme activities of the extracts were also measured. Around 18 compounds were detected in both the methanol and ethanol extract. It was found that the best antimicrobial effect on Gram-negative Pseudomonas aeruginosa was on methanol extract, while the ethanol extract was on Candida albicans fungus (respectively, 2.50mg/ml, 5.0 μg/ml). A 500μg/ml of methanol extract has been shown to have cytotoxic activity high effect on HDF cells. GST and AChE activity were found to decrease in a concentration-dependent manner.Article Biochemical components, enzyme inhibitory, antioxidant and antimicrobial activities in endemic plant Scilla mesopotamica speta(Wiley, 2021) Aktepe, Necmettin; Keskin, Cumali; Baran, Ayşe; Atalar, Mehmet Nuri; Baran, Mehmet Fırat; Akmeşe, ŞükrüIn this study, in vitro antioxidant, antimicrobial, anticholinesterase and phenolic profile of different solvent extracts of Scilla mesopotamica speta were determined in detail. In vitro antioxidant activities and total phenolic and flavonoid contents of plant extracts obtained with different solvents were tested in terms of 2.2-diphenyl-1-picrylhydrazyl (DPPH) free radical scavenging activities. The highest total phenolic and total flavonoid contents were determined in the ethyl acetate extract (62.24 mu g GAE/mg) and chloroform extract (87.72 mu g QE/mg) respectively. The highest DPPH radical scavenging activity was detected in ethyl acetate extracts. Antimicrobial and antifungal activities were investigated by MIC method. The inhibitory activities of the extracts on the acetyl cholinesterase enzyme were investigated. Liquid chromatography (LC) tandem mass spectrometry LC-MS/MS was used to determine the phenolic component content of extracts. Thirty-one different components were identified in the analyses and their amounts were measured. Practical applications Scilla mesopotamica speta is an endemic and medicinal plant. It was determined that the extracts of this plant had a very rich content in terms of phenolic compounds, especially caffeic and ferulic acids. However, this plant was remarkable for its antioxidant, anticholinesterase, and antimicrobial activities. Considering the strong antioxidant, antimicrobial, and enzyme inhibition activities of the Scilla mesopotamica speta it can be suggested as a source of anticancer, antimicrobial, and antiviral drugs.Article Determination of chemical composition and antioxidant, cytotoxic, antimicrobial, and enzyme inhibition activities of Rumex acetosella L. plant extract(Springer int Publ Ag, 2024) Aktepe, Necmettin; Keskin, Cumali; Baran, Ayse; Baran, Mehmet Firat; Atalar, Mehmet Nuri; Keskin, Cumali; Khalilov, RovshanPurpose The phenolic composition, antioxidant, antimicrobial activity, enzyme inhibition activity, and cytotoxic activity potentials of the plant Rumex acetosella L. (R. acetosella) were examined in this study. Materials and Methods: The chemical composition of R. acetosella methanol extract was identified by the LC-MS/MS method. The antioxidant activity was tested using beta-carotene/linoleic acid, DPPH free radical scavenging, ABTS cation radical scavenging, CUPRAC reducing power, and metal chelating activity methods. The cytotoxic activity was determined by the MTT assay using human ovarian adenocarcinoma (Skov-3), glioblastoma (U87), human dermal fibroblasts (HDF), and human colorectal adenocarcinoma (CaCo-2) cell lines. The antimicrobial activity of methanolic extracts was tested on gram-negative (Escherichia coli and Pseudomonas aeuriginosa) and gram-positive bacteria (Staphylococcus aureus and Bacillus subtilis) using the in vitro minimum inhibition concentration method (MIC). Enzyme inhibition activity of R. acetosella methanol extract was measured spectrophotometrically against acetylcholinesterase (AChE) and glutathione S-transferase (GST) enzymes. Results: The findings showed that the major components of the methanol extract content were luteolin-7-O-glucoside (1.599 m/L), polydatin (91,024 m/L), and shikimic acid (0.773 m/L). It was determined that the extract and standard antioxidant (a-tocopherol) results in DPPH center dot, and ABTS center dot + tests performed to determine the antioxidant activity were close to each other, and this value was more effective than the standard antioxidant (alpha-tocopherol) in the CUPRAC test. These results suggested that the plant's antioxidant potential was higher when compared with reference antioxidant compounds. It was determined that the methanol extract of R. acetosella had a weaker effect on the growth of the tested microorganisms than the antibiotics used as standard. The activity of the GST and AChE enzymes was found to be severely inhibited by the methanol extract of R. acetosella. Conclusion: Based on these findings, R. acetosella L. is a medicinal and commercially beneficial plant that warrants further investigation.Article Effect of individual psychoeducation for primary caregivers of children with autism on internalized stigma and care burden: a randomized controlled trial(Taylor & Francis Online, 2023) Bayram Değer, Vasfiye; Gümüş, Funda; Bayram Değer, VasfiyeThis study aims to determine the effects of a six-week individual psychoeducation program provided to primary caregivers of children with autism on stigma and care burden. The study was conducted with 88 primary caregivers (experimental group n ¼ 45, control group n ¼ 43) of children diagnosed with autism. The study, prepared in an experimental design with a pretest, posttest, and control group, was performed between 17 June 2021 and 17 January 2022. In the study, a six-week individual psychoeducation was given to the experimental group. Study data were collected using a personal information form, the Internalized Stigma of Parents in Mental Illness Scale (ISPMIS), and the Caregiver Burden Scale (CBS).The mean pretest ISPMIS total score of the experimental group was X ¼105.756 and the posttest total score after the psychoeducation was X¼66.489,which was a significant decrease (t ¼ 25.571; p < 0.05). The mean CBS pretest total score of the experimental group was X¼61.067 and the posttest total score was X¼31.667, which was a significant decrease (t ¼ 17.499;p < 0.05). The study revealed that the six-week individual psycho education provided to primary caregivers of children with autism decreased the participants’ internalized stigma and perceived care burden.Article Frequency of clinical alarms in intensive care units and nurses’ sensitivity to them: An observational study(American Journal of Critical Care, 2021) Baran, Leyla; Ceylan, Burcu; Güneş, Ülkü YapucuBackground All clinical alarms require nurses to respond even if an intervention is not needed. Nurses are expected to respond appropriately to each alarm and establish priorities among their care practices accordingly. This study was conducted to examine the number and types of clinical device alarms used in intensive care units, the duration of their activation, and nurses’ degree of sensitivity to them. Methods This observational study was conducted in 4 intensive care units in a university hospital in Turkey. A total of 20 nurses (5 from each unit) were observed for a total of 80 hours. The alarms were categorized as valid, false, or technical. Results During the study observation period, the mean number of alarms sounding per hour per bed was 1.8. A total of 144 alarms were recorded, of which 70.8% were valid, 15.3% were false, and 13.9% were technical. The mean duration of alarm activation was 8 minutes for valid alarms, 14 minutes for false alarms, and 53 minutes for technical alarms. Conclusions Nurses’ responses to alarms differ depend-ing on alarm type; for alarms that do not require an emergency intervention, nurses tend to respond late or not at all. (American Journal of Critical Care. 2021;30:186-192).Article Increased DNA Damage and Oxidative Stress Among Silver Jewelry Workers(HUMANA PRESS INC, 2015) Aktepe, Necmettin; Keskin, Cumali; Yükselten, Yunus; Taşkın, Abdullah; Keskin, Cumali; Çelik, HakimSilver has long been valued as a precious metal, and it is used to make ornaments, jewelry, high-value tableware, utensils, and currency coins. Human exposures to silver and silver compounds can occur oral, dermal, or by inhalation. In this study, we investigated genotoxic and oxidative effects of silver exposure among silver jewelry workers. DNA damage in peripheral mononuclear leukocytes was measured by using the comet assay. Serum total antioxidative status (TAS), total oxidative status (TOS), total thiol contents, and ceruloplasmin levels were measured by using colorimetric methods among silver jewelry workers. Moreover, oxidative stress index (OSI) was calculated. Results were compared with non-exposed healthy subjects. The mean values of mononuclear leukocyte DNA damage were significantly higher than control subjects (p < 0.001). Serum TOS, OSI, and ceruloplasmin levels were also found to be higher in silver particles exposed group than those of non-exposed group (p < 0.001, p < 0.001, p < 0.01, respectively). However, serum TAS levels and total thiol contents of silver exposed group were found significantly lower (p < 0.05, p < 0.001, respectively). Exposure to silver particles among silver jewelry workers caused oxidative stress and accumulation of severe DNA damage.Article Induction of apoptosis in human hormone-refractory prostate cancer cell lines by using resveratrol in combination with AT-101(Wiley, 2021) Aktepe, Necmettin; Yukselten, YunusThe aim of this study was to determine the appropriate doses of AT-101 and resveratrol combination in the in vitro hormone-refractory prostate cancer (PC) cell lines, in order to evaluate the cytotoxic and genotoxic effects of this combination on the proliferation of cancer cells, namely PC-3, DU-145 and LNCAP. Cytotoxicity in PC cell lines was analysed by using the XTT Cell Proliferation Assay. DNA damage was performed with the cell death assay. Apoptotic protein levels were performed by Roche Human Apoptosis Array. IC50 values were determined by XTT analysis. The strongest combined doses (100 µM resveratrol + 5µM AT-101) were found to have the strongest synergistic apoptotic and cytotoxic effects on DU-145 cells at 72 hr. While the combined use of resveratrol and AT-101 increased the expression of markers in apoptotic cell pathways on cells, a decrease in the expression of anti-apoptotic markers was detected (p ˂ 0.05). Combined applications of these compounds showed an important synergism in the hormone-refractory PC cell lines, and it was determined that after the post-translational modification, they were significantly effective on the apoptotic pathway. These results have revealed that the combination of resveratrol and AT-101 holds great expectation as a new chemotherapeutic application in the treatment of human prostate cancer.Article Isocarpine as A New Peperidine Alkaloid from Adenocarpus Complicatus with Anticandidal Activity(Ingenta Connect, 2022) Alhilal, Mohammad; Alhilal, Suzan; Gomha, SobhiBackground: Adenocarpus complicatus L. is one of Adenocarpus genus, which has about 50 species, from Fabaceae (Papilionaceae) family. This plant is found in the Mediterranean basin as a southern west part of Europe and North Afric forestes. Objective: Isolation of Isocarpine From Adenocarpus Complicatus as Anticandidal Agent. Methods: A new piperidine alkaloid Isocarpine was isolated from the aerial parts of Adenocarpus complicatus L., which is gross in Syria. The structure of the new compound was determined by UV, IR, EI-Ms, 1H-NMR, 13C-NMR, DEPT-135, DQF-COSY and HMQC. Results: The anticandidal activity of isocarpine was screened against 38 Candida strains and showed remarkable inhibitory effect compared with fluconazole as an antifungal reference drug. Conclusion: Isocarpine is antifungal agent.Article Outcomes of genital hygiene and anxiety training for mothers of girls with profound intellectual disabilities: A randomized controlled experiment(Taylor & Francis Online, 2022) Bayram Değer, Vasfiye; Bayram Değer, Vasfiye; Sakız, HalisFemale children with profound intellectual disabilities (IDs) may experience symptoms of urinary tract infections (UTIs) and depend on others’ care. However, their caregivers may lack general hygiene skills and experience heightened anxiety when their care is expected. This study reports outcome of a training that aims to enhance genital hygiene skills and decrease anxiety levels of mothers of girls diagnosed with profound ID. The study was conducted in a city located in Turkey in 2020. It was designed with a randomized controlled experimental approach based on a pre-test and post-test model with experimental and control groups. The sample consisted of 66 mothers of girls who were diagnosed with profound ID, did not have UTI but were at high risk of developing it. A 6-week program with 24 sessions was implemented. After the training, mothers in the experimental group changed the sanitary pad and the underwear during menstruation and gave their daughters a bath more frequently; had a higher knowledge of recognizing and preventing UTI symptoms and cleaning of the perineum area; and had significantly lower levels of anxiety. A carefully designed simulator-based training can enhance the knowledge and skills of mothers to recognize the UTI symptoms, apply their knowledge to prevent the symptoms and implement genital hygiene practices, which in turn have a positive effect on reducing their level of anxiety.Book Review Seven-membered Rings with Three Heteroatoms: Chemistry of 1,2,5-and 1,4,5-Thiadiazepines(BENTHAM SCIENCE PUBL LTD, 2018) Doğru Mert, Başak; Elattar, Khaled M.This review describes the chemistry of 1,2,5- and 1,4,5- thiadiazepines. The investigated heterocycles are monocyclic and fused 1,2,5- and 1,4,5- thiadiazepines. The different sections cover: structural elucidation "spectroscopy", reactivity of substituents attached to ring carbon and nitrogen atoms, synthesis of monocyclic and fused heterocycles incorporated 1,2,5- and 1,4,5- thiadiazepine moiety. Nitrogen and sulfur atoms of thiadiazepine ring system have the ability to donate the electrons pairs in order to form coordination bonds forming different complexes. The aim of this review is to discuss the chemistry of title compounds up till now. The reaction mechanisms are described and discussed, as well as, the biological applications of the target compounds.Article Synthesis and biological evaluation of new aza-acyclic nucleosides and their hydrogen complexes from indole(SpringerLink, 2022) Alhilal, Suzan; Alhilal, Mohammad; Gomha, Sobhi M.; Ouf, Salama A.Three novel aza-acyclic nucleosides and two hydrogen complexes were isolated by flash chromatography after being produced in a reaction between indole and dibenzosulfonyl diethylamine (which had previously been prepared) in the presence of sodium and absolute ethanol as a basic catalyst. Structures of new compounds and complexes were determined by 1D-NMR: 1H NMR, 13C NMR, DEPT-135, 2D-NMR: COSY, HMQC, HSQC, HMBC, IR, and MS spectroscopy. The synthesized compounds were evaluated against a wide range of microorganisms, including Gram-positive and Gram-negative bacteria as well as fungal strains. These compounds showed good biological activity.Article Synthesis of Novel Acyclic Nucleoside Analogue Starting From 6-Aminouracil as Potent Antimicrobial Agent(Polycyclic Aromatic Compounds, 2021) Alhilal, Mohammad; Sulaiman, Yaser A. M.; Alhilal, Suzan; Gomha, Sobhi M.; Ouf, Salama A.6-Aminouracil and 2-bromoethyl amine were prepared, as starting materials to be introduced as an alkylating reagent with sodium carbonate as a catalyst. Acyclic nucleoside was prepared for the first time, the expected structure of the final new compound 3 was determined based on IR, NMR, and mass spectroscopy, with safe and mild reaction conditions. The synthesized acyclic nucleoside has a potent and efficient antimicrobial activity compared to reference drugs particularly as an antibacterial agent, and can be used as an alternative to the commonly used antibiotics after performing the necessary biological research for its validation.Book Türkiye’de Acil Sağlık Hizmetleri(Nobel Akademik Yayıncılık, 2023) Bütün, Ahmet; Bütün, Ahmet; Konakçı, GülbinSevgili okurlar, “Türkiye’de Acil Sağlık Hizmetleri” kitabı çok kıymetli bilim insanlarının ortak çabasıyla oluşmuş nadide bir eserdir. Bu kitap, Türkiye’de acil sağlık hizmetleri konusunda geniş bir bakış sunmayı amaçlamaktadır. Ülkemizde acil sağlık hizmetleri, sıklıkla hayat kurtarıcı niteliktedir. Ancak bu hizmetlerin sunumu, yönetimi ve koordinasyonu oldukça zor ve karmaşıktır. Kitapta, acil sağlık hizmetlerinin politikalarından afet yönetimi ve triyaj gibi konulara, geriatrik hasta yönetiminden obstetrik acillere kadar geniş bir yelpazede konular ele alınmaktadır. Ayrıca acil servislerde liderlik, iletişim, yapay zekâ gibi çağdaş konular da tartışılmaktadır. Bu kitapta yer alan bölümler, acil sağlık hizmetleri sunumu ve yönetimi konusunda farkındalık yaratmayı, bu alanda çalışanların ve öğrencilerin bilgi düzeyini artırmayı hedeflemektedir. Ayrıca okuyucularımızın bu konuda kendilerini geliştirmelerine ve acil sağlık hizmetleri konusunda daha etkili bir şekilde çalışmalarına yardımcı olmak amacıyla hazırlanmıştır. Kitabın hazırlanmasında emeği geçen tüm yazarlara, editörlere ve çalışanlara teşekkür ediyoruz.
